Social Security in Spain is a public system that guarantees healthcare, pensions, allowances, and other benefits to everyone residing legally in the country. Healthcare is considered a fundamental right and is provided equally to all residents, including citizens of the European Union.

What is the SIP card?The SIP card (Sistema de Información Poblacional) is a personal and non-transferable document that grants access to the Spanish public healthcare system, including primary care, specialist care, and hospital care. The card also enables the management of medical records and prescriptions.

Requirements for a European citizen to obtain the SIP card

  • Legal residence: You must be registered in the population register (empadronamiento) of your municipality.
  • Social Security registration:
    • If you work in Spain, you need a Spanish Social Security number.
    • If you are retired in another EU country, you can use the S1 form.
  • Required documents (depending on the situation):
    • Pensioners: Passport or ID card, proof of registration or residence, pension certificate, S1 form, and a sworn/legalized birth certificate.
    • Employees: Passport or ID card, proof of registration, Spanish Social Security number, work history (vida laboral), and an S1 form if applicable.
  • Application: Submit your application at your local health center (centro de salud), providing original documents and copies. The card is usually issued immediately or sent by mail.
  • Costs: The SIP card is generally free of charge. In certain cases (such as temporary cards), a small administrative fee may apply.
